The company was founded by healthcare executives and attorneys who understand, from years of personal and professional experience, the trauma experienced when a patient’s healthcare choices are not clear, or are unknown.

As a result of these experiences, they developed Embark’s Advance Directive Solution (ADS), a comprehensive online and telephonic resource with all the information and legally current forms to create an enforceable advance directive. With the ADS, Embark ensures:

• Advance Directives are legally accurate and completed correctly, based on the most current forms and instructions available for a given state.
• Properly completed Advance Directives are stored electronically in the Embark Health Advance Directive Solution repository.
• Through Embark’s extensive marketing and public relations outreach, hospitals, physicians and health plans are made aware of the existence of the Advance Directive Solutions so that they can obtain the
  patient’s advance directive when needed.
• The National Legal Support Network is available to verify that the directive is legal and accurate.

To further assist families during the difficult times when a family member is ill or has passed away, Embark also is working hard to offer the Personal Legacy Solution. The Personal Legacy Solution (PLS) will be a secured, confidential, electronic repository for tracking the location of assets, the location of other important items or documents, and for storing important messages to loved ones. The PLS makes the stored information retrievable in accordance with the individual member’s specifications.

Embark also works with health plans and healthcare providers to make the Advance Directive Solution and (soon) the Personal Legacy Solution available to their members and patients. Simple, easy, and affordable – Embark has
